ACP Ambassador Training

Understanding how health care decisions are made, in Ontario, is critical to our work.  Helping clients have conversations around Advance Care Planning (ACP) is everyone’s responsibility!

Objectives

Designed as a train-the-trainer model, this program focuses on building the capacity across diverse community sectors in Waterloo-Wellington (i.e., HR, health care, community organizations, etc.). Eligibility Criteria

This training is essential for anyone who:

• already has a basic knowledge of ACP in Ontario and wishes to enhance their understanding. If not already done, please review the following:  videos, prior to class:
  • Substitute Decision Maker Video
  • ACP 101 Video
• works directly with clients/patients/general public who interact with the health care system
• Manages/works closely with colleagues and is responsible for knowledge transfer within their agency/organization

Attendees will learn the necessary skills to :

• initiate correct ACP conversations with clients/patients
• build staff/colleague capacity to initiate correct ACP conversations
• deliver ACP education/presentation(s) within their own organizations

Please Note: those previously trained in LTC may take Ambassador Training as a refresher/update
