HPC Consultation Services has received funding from the Waterloo Wellington Local Health Integration Network (WWLHIN) to work with the Long Term Care (LTC) Homes in Waterloo Wellington (WW) with an aim to increase the knowledge, understanding and capacity to deliver a Palliative Approach to Care. This initiative is designed to move towards ensuring that residents and families living and dying within a WW LTC setting receive the same access to excellent best practice palliative care as residents who die in the community, residential hospices, or other health care sector locations.
The overarching strategy for the WW Quality Palliative Care in Long Term Care initiative is to incorporate a collaborative and integrated approach to education and training related to all aspects of palliative care including Advance Care Planning, Pain & Symptom Management at end of life, and the Hospice Volunteer program.
